Dear Patients,
The purpose of writing this letter to you is to inform you that, due to health problems, I will be retiring from the practice of medicine June 30,2012. I am very sad to be retiring several years before I had anticipated, and to give up the rewarding privilege of being your doctor, and helping bring your babies into the world.
I’ve needed time to come to terms with all of this, and wanted to write once I had enough information to help ‘deliver’ you into the care of another medical practice. Understanding the difficulties this may pose for you, I at least wanted to be able to give you my recommendations, and make this entire transition as easy for as possible.
Elizabeth Sivesind, my Physician Assistant of nineteen years, would love to keep seeing you if you are currently her patient; or, if any of my patients would like to see her, they are welcome. Many saw both of us over the years. She will be working with Yaron Friedman, MD, an Ob-Gyn two blocks away at 130 La Casa Via in Walnut Creek. You may call (925)301-9875 now to make appointments beginning July 23, 2012. (She’ll be on vacation the first three weeks in July.)

I also recommend doctors Judith Hartman, Janine Senior, Janette Walker, Sari Kasper and Christine Riley. Dr. Riley is a solo practice physician, while the other doctors are all part of Ob-Gyn Specialists, and you may discover more by visiting their websites. They have been in my call group for the past 8 years, and they are all excellent physicians.
If you are going to be seeing Liz Sivesind, PA-C, you do not need to sign a record transfer. If you wish to transfer, our office will be pleased to send a secure email of your records in PDF format at no charge to the practitioner of your choosing. Or, you may choose to have your records emailed to you, which you can access with your date of birth as the password. (For example, November 20, 1960 would be: 112060.) We will need a signed authorization to transfer records. Enclosed is a sample form we need to receive from you my mail or fax as soon as you have the information. If you wish a copy emailed to more than one office, or a copy after August 31, 2012, there will be a charge of $25 each.
If you need a prescription refill, you must request it by June 15, 2012, and I will extend it 3 months if appropriate. Our mailing address after June 30, 2012 will be 1839 Ygnacio Valley Road, #404, Walnut Creek, CA 94598.

Dr. Craig, Thank you so much for taking such great care of me and bringing my babies safely into the world! You made me feel comfortable every step of the way and I always enjoyed my office visits and seeing you and staff! I will miss you and I wish you rest, peace, happiness and health in your retirement!
